Split-thickness skin graft

Why are we operating?

Borrowing the top layers of skin from a healthy site to resurface a wound that cannot close itself; the graft survives on the bed it lands on, which is the whole art.

Indication: Burns after excision, large wounds with a healthy granulating bed, donor coverage after flap harvest.

  • Honest area assessment is treatment: the fluid formula inherits every charting error, in both directions.
  • Circumferential deep burns are compartment syndromes: the escharotomy done possibly-early beats the one done certainly-late.

The operation in 4 steps

  1. 01Preparing the recipient bed

    The wound is debrided to healthy, bleeding, uninfected tissue; haemostasis is meticulous.

    Why: A graft has no blood supply of its own for days: it drinks plasma, then waits for capillaries to grow in. A bed of slough, dead tissue or haematoma starves it, and the commonest cause of graft failure is the bed, not the graft.

  2. 02Harvesting the graft

    A dermatome shaves a calibrated split-thickness sheet, classically from the thigh, leaving the donor dermis behind.

    Why: Split thickness is the trick that makes the donor site self-healing: the deep dermis left behind regrows epidermis from its adnexal structures within a fortnight, so skin is borrowed rather than spent.

  3. 03Meshing and application

    The graft may be meshed to expand it and let exudate escape, then it is laid on the bed, trimmed, and fixed at its edges.

    Why: Meshing multiplies coverage and drains the fluid that would otherwise lift the graft off its bed; the interstices heal across in days. Contact and stillness are everything from here.

  4. 04Dressing and immobilisation

    A pressure dressing or vacuum bolsters the graft; the part is immobilised, with the first inspection deferred for days.

    Why: Shear is the third killer of grafts after haematoma and infection: a graft that slides even a little tears the capillary ingrowth that was feeding it. The dressing's job is contact without movement.
