VATS lobectomy
Why are we operating?
An anatomical lobe of lung removed through ports: artery, vein and bronchus each isolated and divided, nodes cleared, cancer out inside a bag.
Indication: Early-stage lung cancer with adequate lung function; the fitness assessment is half the decision.

The operation in 4 steps
01Lung isolation and access
One-lung ventilation drops the operative lung; ports and a utility incision are placed without rib spreading.
02Hilar dissection
The lobar vein, artery branches and bronchus are individually dissected clean in the fissure and at the hilum.
03Dividing vein, artery, bronchus
Each structure is stapled and divided in sequence, then the fissure completed and the lobe freed.
04Nodes, retrieval, drain
Mediastinal and hilar nodal stations are sampled or dissected, the lobe leaves in a retrieval bag, a chest drain finishes.
Anatomy you need
The bronchial tree is the map of lobectomy: each lobar bronchus and its segmental branches are the structures stapled.
What can go wrong
Postoperative bleeding
Tachycardia first, hypotension late; falling urine output; a distending abdomen or rising drain output.
Atelectasis & pulmonary complications
Low-grade fever and mild desaturation in the first 48 hours, especially after upper-abdominal incisions.
Venous thromboembolism
Unilateral leg swelling, or sudden dyspnoea, pleuritic pain, tachycardia, sometimes the only sign is unexplained hypoxia.
Surgical-site infection
Wound erythema, warmth, discharge, and pain out of proportion, typically from postoperative day 4–7.
